to join our experienced team, to provide intensive case
management and support to isolated older people living in
their own homes.
The attributes we are looking for include:
• Relevant work experience and interest in supporting older
• Appropriate qualifications in Community/Social Work
• Knowledge of the health system in relation to care for the
• A knowledge of, and a willingness to work with Strength
• Proven ability to develop plans and support people to
reach their goals
• Ability to establish and maintain community and volunteer
• Experience in working with older people with alcohol and
drug addictions and/or mental illness, is an advantage.
